Punchy?
This build's main form of damage is Furious Punch - why Furious Punch you may say? Because it is quick. And when you have activated Hulk Mad you want to hit as many times as you can while it is active (also the relative damage of all punches is roughly equal, when you take attack speed into account).
Spirit & Anger
The build centers around building anger and then spending it on Unbridled Rampage (for teh heals); and using spirit on Hulk Tough/Free/Mad.
Crit like a Boss?
Critical chance is buffed by Worldbreaker and Hulk Mad (for a total of 47% base chance).
Critical damage is buffed by Worldbreaker .
HULK SMASH
Movement skill is Avalanche leap - which is great for gathering mobs up ready for HULK SMASH WITH FISTS! Earthquake Leap is a freebie because it is a prerequisite to Avalanche Leap .
Two points are used to get Destructive Mayhem , but I don't see this is crucial and the other points could be put into Short Temper or Hulk Free - the choice would depend on whether you want more move speed, or health and defense.
Defense
Hulk Tough was prioritised over Short Temper because the defense bonus is greater - and the damage reduction + defense trumps the additional health (you can augment +health% on costume anyway).
You could take some of the points from Hulk Free and put towards Short Temper - it depends how much you value movement speed (or how much is on your gear ;) ).

NB. I would probably use Juggernaut [item]36[/item] if I didn't already have invulnerability orb on one of my Cosmic items.

Edge of Infinity [item]75[/item] - for MOAR crit chance and crit damage.
Metasensor Array [item]100[/item] - for attack speed and defense penetration.
Costume Core:
Hulk DEMANDS +1 Fighting, Special item find and damage% to health (life on hit) [item]197[/item].
